Transfer/Claim Forms

Election Form for Transfer of Accrued Benefits		For you to transfer the MPF benefits.
Transfer of Accrued Benefits with Outstanding Contributions & Guide on How to Fill in the Form		For request of transfer of accrued benefits with outstanding contributions per Section 156 of the MPF Schemes (General) Regulation, subject to obtaining written consent by the MPFA.
Claim Form for Payment of Accrued Benefits		For you to claim the MPF benefits.

In processing a claim for payment, claimant is required to produce evidence in accordance with the reason for withdrawal. Claimant may use the following declaration form to make statutory declaration for the evidence of claim.

Statutory Declaration for Claims for Payment of Accrued Benefits on Grounds of Early Retirement		An original copy of a certified true copy of the statutory declaration form on early retirement.
Statutory Declaration for Claims for Payment of Accrued Benefits on Grounds of Total Incapacity		A medical certificate issued by a registered practitioner.
Statutory Declaration for Claims for Payment of Accrued Benefits on Grounds of Permanent Departure from Hong Kong		An original copy or certified true copy of the statutory declaration on permanent departure.
Statutory Declaration for Claims for Payment of Accrued Benefits for Small Balance Account		An original copy or a certified true copy of the statutory declaration on small balance account.
Statutory Declaration on Cessation of Employment		An original copy or certified true copy of the statutory declaration on cessation of employment.
